Financial Policy

Based upon the insurance information provided on the day the appointment(s) was made, we will assist you in the request for a referral or authorization, according to the rules of your insurance. If we are notified of a denial prior to your appointment, we will contact you. Your insurance company will be sending you the same information. If the referral is not gained by the date of the appointment, you may reschedule the appointment or you may sign a waiver to accept financial responsibility for that day's charges.

After the visit, we will bill your primary insurance. And as a courtesy, we can bill your secondary insurance carrier if you have provided the information when you registered for your appointment.

If you owe anything after your insurance company(s) have responded, we will send you a bill. All bills are due upon receipt and may be paid by check, cash, VISA, or Mastercard.  You can also pay online.  The statement shows which insurance we have billed for you. If there is an error, please mail a copy of the front and back of your insurance card with a request to bill the correct insurance carrier. You may also fax the corrected info and request to our Business Office at 971-224-2477.

Medicare  (Our office does accept Medicare Assignment, which requires us to submit the Medicare   claim on your behalf, and the Medicare payment is sent directly to our office. After Medicare has paid, we will send you a statement with your balance. )
